What are Templates?
Templates are a combination of Elements that you create to be used for data collection. They can be customized by you, and can be used for things such as Body Weight, compliance forms, athlete history, etc. The following guide is a basic look into how to create a Template - if you have any questions, please contact your local support team.
How do I create a Template?
To create a template, you need to create pages and drag Elements into the appropriate Rows and Columns (for a list of Element types, please click here).
- Navigate to the Forms module > Templates and click the "+ Create Templates" button in the top right hand corner of the page.
- To title your form, scroll to the bottom of the page and find where it says "Untitled Template." You can change this text to reflect the name of the Template.
- Create a Page for the Elements to fall onto by clicking the "Add Page" button and naming your page appropriately.
Note: Pages cannot be re-organized, so make sure your pages are in the order you'd like when you create them. - From here, you can begin creating or adding your elements. To create an Element in the Templates screen, click on the "+" button in the Add Elements tab and create your Element based on the type. Where necessary, be sure to add the placeholder text and options.
If you've already created your Elements in the Elements module of the Left Hand Menu, you can search for them simply by typing into the search bar in the Add Elements page. - Once your elements are created, you can begin adding them onto the Template by dragging them from the Add Element menu on the left over to the Preview on the right.
- To continue to build out the Template, you'll need to add more Rows and Columns. There are two ways top do this:
- Drag the Rows/Columns from the Add Elements tab on the left-hand side onto the portions that highlight green when you hover over them.
- Click on the green space that highlights when you hover over it. If you look closely, there is a + symbol in the middle of the box.
Note: If you'd like to delete a Row/Column (or an element, but leave the Row/Column where it is), click and hold the grey header in the top left hand corner of the Row/Column and drag it towards the bottom of the screen. As you begin to drag, a red Trash icon will appear for you to drag it over.
- Drag the Rows/Columns from the Add Elements tab on the left-hand side onto the portions that highlight green when you hover over them.
- In order to edit the Element itself (to make it required, to add a Max/Min, to change display name or placeholder text, etc.), simply click on the Element in the Preview on the right. It will be highlighted in orange, and the Element Settings will show on the left hand side of the screen.
Note: In order to show an element to those that are entering data, the Element must be Enabled. If you'd like to learn how to set up a Trigger, please click here. - Under "General" on the "Add Elements" tab, you'll see that there are two other options besides Rows and Columns - Text and Media.
- Text will add messaging that the user entering data will have the ability to view/read, but not change.
- Media will allow you to add things such as Images for logos, videos, etc.

- To check your work and see what the Template will look like from the User's perspective, click on the Preview button in the bottom right hand corner of the screen. Click it again to return to the editing screen.
- Once you feel comfortable with your Pages and Elements, click the orange Save button in the bottom right hand corner. This will add the Template to your Forms list.
