What are Forms?
Forms are used to capture data for athletes and to create a holistic view of information surrounding your athletes. Compared to the first version of Data Capture, which needed to be configured by members of Catapult's staff, Forms will allow anyone with the proper permissions to create data capture templates for themselves in order to enter data. Data can be entered in a variety of ways by both team staff as well as the athletes themselves.
How do I access Forms?
There are three pages available via the Forms Module - Elements, Templates, and Collections. For a further look into each of these, please click on the respective blue embedded links below:
- Elements
- How each individual piece of data will be collected.
- Templates
- The forms that you've created - these are composed of elements.
- Collections
- Allows you to connect multiple forms into a single data collection making it more even more efficient if you're needing to collect multiple forms for an individual at once.
What does a template look like?
They can be customized to fit your needs - forms that you have on paper or in PDF's that are currently being used to collect information from athletes (Wellness Questionnaire, RPE, Body Weight, etc) or on behalf of athletes (DXA information, Medical History Questionnaire, etc) can be converted into a form for use. Data can be collected on the web as well as in our mobile app.
